In consideration of and as a condition of the listed player’s (the “Player”) acceptance and participation in a futsal/soccer activity to be played at Mountain Brook Presbyterian Church, Hope Community Church, Brookwood Forest Elementary School, Mountain Brook Elementary School, Cherokee Bend Elementary School and/or Crestline Elementary School and recognizing the possibility of physical injury associated with playing futsal/soccer, I hereby for the Player and myself listed, our heirs, executors and administrators, waive and release, discharge and/or otherwise indemnify Forest Futsal LLC and its associated personnel, including the owners of facilities utilized for futsal/soccer against any claim by or on behalf of myself or the Player resulting from the Player’s participation in any activities at Mountain Brook Presbyterian Church, Hope Community Church, Brookwood Forest Elementary School, Mountain Brook Elementary School, Cherokee Bend Elementary School and/or Crestline Elementary School that is now existing or hereafter may exist for damage or injury to the Player, or to any person or property, resulting from the negligence or other acts of any employees or volunteers in connection with the Player’s participation at said facilities. I further agree to indemnify and to hold Forest Futsal LLC, Mountain Brook Presbyterian Church, Hope Community Church, Mountain Brook Schools (including their officers and associated personnel) free and harmless from any loss, liability, damage, cost or expense which they may incur as a result of any injury and/or property damage that the Player or I may cause or sustain while participating at Mountain Brook Presbyterian Church, Hope Community Church, Brookwood Forest Elementary School, Mountain Brook Elementary School, Cherokee Bend Elementary School and/or Crestline Elementary School. I further agree that this waiver, release and assumption of risks shall be binding on the heirs and assigns of the undersigned and the Player. I understand that medical insurance coverage is necessary and required for the Player’s participation at Mountain Brook Presbyterian Church, Hope Community Church, Brookwood Forest Elementary School, Mountain Brook Elementary School, Cherokee Bend Elementary School and/or Crestline Elementary School and that adequate coverage is the responsibility of the parent or guardian of the Player. I HAVE CAREFULLY READ THIS RELEASE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ME AND THE PROGRAM AND SIGN IT OF MY OWN FREE WILL. THIS FORM MUST BE SIGNED BY EACH PLAYER AGE 18 OR OLDER OR BY THE PARENT OF ANY PLAYER UNDER AGE 18.

I further agree that this waiver, release and assumption of risks shall be binding on the heirs and assigns of the undersigned and the Player. I understand that medical insurance coverage is necessary and required for the Player’s participation at Mountain Brook Presbyterian Church, Hope Community Church, Brookwood Forest Elementary School, Mountain Brook Elementary School, Cherokee Bend Elementary School and/or Crestline Elementary School and that adequate coverage is the responsibility of the parent or guardian of the Player. Recognizing the possibility of physical injury associated with futsal/soccer and in consideration for Forest Futsal LLC accepting registrant for its futsal/soccer training, camps, and events, I hereby release, discharge, and/or otherwise indemnify Forest Futsal LLC, its affiliated organizations and sponsors, their officers, volunteers, and associated personnel, including the owners of the fields and the facilities utilized, against any claim by or on behalf of my child's participation in any and all Forest Futsal LLC events and/or being transported to and from the same. My child has received a physical examination by a physician and has been found physically capable of participating in the sport of futsal/soccer and by extension this tournament. I hereby give my consent to have an athletic trainer and/or doctor of medicine or dentistry provide my son/daughter with medical assistance and/or treatment. I also assume the financial responsibility for any medical treatment for my child.

In consideration of and as a condition of the listed player’s (the “Player”) acceptance and participation in a futsal/soccer activity to be played at Mountain Brook Presbyterian Church, Hope Community Church, Brookwood Forest Elementary School, Mountain Brook Elementary School, Cherokee Bend Elementary School, and/or Crestline Elementary School, I understand that there may be individual and team photographs and video taken of the players at Forest Futsal LLC and its events. These pictures and videos may also be displayed in local newspaper, newsletters, and the Forest Futsal LLC website. I give Forest Futsal LLC permission to photograph and video my son/daughter individually and as part of the futsal/soccer activity, knowing that this picture or video may be used for the purposes described above and for official Forest Futsal LLC press releases.

In consideration of being allowed to participate in any way in any Forest Futsal LLC related events and activities I, the undersigned participant, parent, or legal guardian, acknowledge, appreciate, and agree that: By participating in Forest Futsal LLC related events and activities, there are certain risks to me arising from or related to possible exposure to communicable diseases including, but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for the Coronavirus Disease (also known as COVID-19) and/or any mutation or variation thereof (collectively referred to as “Communicable Diseases”). I am fully aware of the hazards associated with such Communicable Diseases and knowingly and voluntarily assume full responsibility for any and all risk of personal injury or other loss that I may sustain in connection with such Communicable Diseases. I, for myself or for my minor child(ren) or ward(s), and on behalf of my/our heirs, assigns, beneficiaries, executors, administrators, personal representatives, and next of kin, HEREBY EXPRESSLY RELEASE, HOLD HARMLESS, AND FOREVER DISCHARGE Forest Futsal LLC, Mountain Brook Presbyterian Church, Hope Community Church, Mountain Brook Schools (including their officers and associated personnel) (the “Released Parties”), from any and all claims, demands, suits, causes of action, losses, and liability of any kind whatsoever, whether in law or equity, arising out of or related to any ILLNESS, INJURY, DISABILITY, DEATH, OR OTHER DAMAGES incurred due to or in connection with any Communicable Diseases, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES OR OTHERWISE, to the fullest extent permitted by law. I agree that this Agreement is intended to be as broad and inclusive as is permitted by the laws of the State of Alabama, and if any portion hereof is held invalid, it is agreed that the remainder shall continue in full legal force and effect. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
